We complete a comprehensive assessment to gather the information and observations across many functional domains (daily activities) to provide clinical recommendations regarding funding and support requirements.
We provide assessments to determine if a equipment or device can improve function and enable the person to do the everyday things that are important to them. We can also provide advice on AT for reducing risk of manual handling injury, pressure injury risk, reducing risk of falls, or enabling independence and quality of life.
We can provide tailored cognitive assessments. We also provide advice on compensatory strategies, and education on how to manage symptoms.
We will complete a comprehensive FCA assessment to support your home and living application, but will provide more detailed clinical recommendations on support needs, eligibility, housing type requirements, and information on how the participant lives with others.
Do you have a skill you wish to learn? or a goal to become independent in a certain activity? Do you want to live independently or move out of home? We can provide person to person sessions, tailoring capacity building and skill development unique to you and your specific requirements. This includes activities such as cooking, budgeting, shopping, catching public transport, chores, laundry, or self-care such as brushing teeth to name a few.
We can complete minor modification assessments and provide reports, recommendations or drawings for minor modifications that are under the price of $10,000. This includes grab rails, changing flooring and other AT items such as toilet aids, rubber/portable ramps as such.
We can identify manual handling injury risk, provide advice on appropriate equipment to reduce risk and provide training to family and support workers on use of client specific equipment.
Behavioural activation is an approach to mental health that involves someone using behaviours to influence their emotional state. It is a practical and evidence-based treatment for depression. This involves individualised Capacity Building/therapy in a person to person capacity.
We can provide support letters to address housing requirements, waitlists, functional needs, risk identification, and minor home modifications. This will usually require a FCA assessment.
